the vre killer

An oxazolidinone that inhibits protein synthesis. Used for resistant Gram-positives where Vancomycin fails or is not tolerated. Bioavailability: 100% (IV dose = Oral dose). Excellent tissue penetration (Lung/CNS).

indications

  • VRE (Vancomycin Resistant Enterococcus): First line.
  • MRSA Pneumonia: Superior lung penetration compared to Vancomycin.
  • Necrotizing Fasciitis: Toxin suppression (theoretical benefit).

adverse effects

  • Myelosuppression: Thrombocytopenia/Anemia. Risk increases after 14 days. Weekly CBC required.
  • Serotonin Syndrome: Weak MAO inhibitor. Caution with SSRIs/TCAs/Fentanyl.
  • Neuropathy: Peripheral/Optic neuropathy (long term use).
  • Lactic Acidosis: Mitochondrial toxicity.
